Why didnt Madara and Izuna, and Sasuke and Itachi.... Just trade eyes

The answer is very simple~

Madara didn't know anything about the eMS before he got blind. Madara was the first one who revealed the secret of "the new light" after he took Izunas eyes-according to Itachi.
And you have to exchange an eye that holds the power of the Mangekyō. That's why Itachi and Sasuke coudn't just trade their eyes. Itachi would have died anyway due to his illness.
