So I know a lot of people have already thought about it, but this is just my take on it.
I think that Sasuke and Naruto will go into the after life, or whatever and meet the sage via Naruto's sage chakra.
Once that happens they will probably get a speech about how they're supposed to beat madara (who has now become the Juubi Jinchuuriki) and how they need to work together. With some flashbacks of course.
Then, I think that both will get the older and younger son's powers in order to work together and beat madara. They'll beat him, but afterwards Sasuke will try and take on Naruto.
He'll do it because of the same reason that the elder son did it:jealousy. When the sage spoke to them he told Sasuke that Naruto would be the savior, and since Sasuke has seen how the past hokage's rein went, he'll think that Naruto would just be repeating it.
So he'll be jealous and angry that the Sage chose Naruto over him and challenge naruto. They will fight an awesome battle where naruto will eventually win. To end it all, Naruto will give Sasuke the TNJ and convince him that he is the right one for the job, but Sasuke will have to work with him- something like how Hashirama asked Madara- and they'll live happily ever after.
