This can be due to the following reasons

• She knew katsuyu for a long time
• Shizune is her summoner
• Katsuyu respects her because she is tsunade disciple
• Shizune was trained by katsuyu in sage mode

shizune most likely trained with katsuyu. Because katsuyu refers to her as sama which means a highly respectful title
Sakura trained with katsuyu because how will katsuyu know about sakura seal if she never worked with her before.​

Overall katsuyu still have more respect for katsuyu and Tsunade.

This could be, but to take it even further, Shizune has a prestigious status of being the head of the medical corps, while Sakura is still a practitioning Chuunin. At the tender age of 16.

I think it is because Sakura is a child (she only 16), and Shizune is a rather grown woman (30-ish) and because Katsuyu have know Shizune from a child until she was an adult. She is probably like an cool aunt type to Shizine.
as for Sakura, she is one of the most respectful people in the series (to people who she don't know and to authority - unless she needs to put them in their place) so she refers to everyone as Sama, san,chan, My lady, ect. Katsuyu is like a big sister to Sakura, just like Tsunade it only make sense that she addresses her as she would one would a student...

She was also proud of Sakura for mastering the seal too...
I think she when address Sakura as her superior when she matures more ( in wisdom, power, and physically) and/or truly surpasses Tsunade
