This is just a random theory I thught of and it's probably not right but I'm going to throw it out there. When we last saw shisui he was telling itachi to take his other eye and that danzo was coming for it. This is probably around the same time that danzo was on his sharingan hunt and was working with orochimaru.
So the theory is that shisui didn't die, he gave his eye to itachi and then danzo came for him, found him with no eyes and for whatever reason, kept him alive. Shisui went throug the same expirement as danzo and got the sharingans on his arms. I'm just trying to think of a way he could have survived since the only person that would be rustled that itachi was dead is shisui. Anyways it's probably off and it might not even be shisui but I thought I'd throw this out here
