But Sasuke didn't even really kill Itachi. He only unlocked MS because the truth of his brother's life devastated him. He probably would not have unlocked it if Obito didn't tell Sasuke that story.

Sasuke thought he did back then :I, in his point of view he killed someone closest to him, his own brother which he hated with a passion by the way, I believe he would have awakened it, the only differences is, in his view he killed itachi, and avenged the clan instead of learning the truth

I believe there is something about Shisui's Mangekeyo we don't know. If you compare all the Mangekeyo's, they all resemble a sort of triple pattern, while Shisui's portrays a four way symbol.

Also, his cast the most powerful genjutsu, so maybe his Mangekeyo was obtained in a way known only to him, which is why his was so unique.

We may never know how Shisui obtained it.

He got it before the massacre, by aiding Shisui's death. The wiki has the exact wordings.

However, being an ANBU, Shisui had the ability to make his body disappear without a trace; therefore, nobody could get solid evidence. Kabuto talks about how he wanted to resurrect him, but couldn't because of the lack of DNA.
