its modeled after this statue i think called "warriors of tengu" though the beak on sasuke's susano'o is much more pointed, but it is the same design. they even have the big jewel on their forehead so yea. google it for your self

It's a hawk. Sasuke was a snake but now is a hawk and his Susanoo is after Warriors of Tengu and a Hawk design mixed.

Its not a hawk, its a crow. Hawks have shorter beaks and its a susanoo from Sasukes and Itachi's eyes, and Itachi's trademark was the crow
