Plus the fact that throughout the manga eye transplants are fully active and healed within seconds of the transplantation, but Sasuke had to rest and heal for multiple days or even a week when he gained EMS.

So clearly the transformation from MS to EMS takes a far greater toll on the body than any other eye implant, which suggests a greater process and transformation than simply putting in a different pair of eyes and letting your chakra mix within it.
To be fair Madara (whom i asssume you're referring to) isn't your average person. When he got his right rinnegan he had hashi's regen and lifeforce. The 2nd and 3rd times he was a juubi jin.

But back to the main point, how would they fuse into one? The manga on multiple occassion says transplant, not implant. Do they magically disappear?

Well yea he'll call em Itachi's eyes. He have his eyes too. Same reason Madara have Izuna's.

Even the "demon" have 4 Sharingan eyes and a empty Rinnegan on its forehead. I highly doubt taking out your eyes and implanting your siblings would give you an "eternal mangekyou" if that's a symbolism I showed then it holds more merit since there is nothing in the manga that indicates that it's a replacement.
Dude, the manga made it clear that Sasuke transplanted Itachi's eyes and Madara transplanted Izuna's, it's why the eyes have the original owner's pattern. The Sharingan chakra produced by Sasuke's brain combined with the Sharingan chakra in Itachi's eyes fuse to create EMS, the demon symbolizes the union of two Sharingan chakras.

I wasn't... I was referring to literally every eye transplant in the manga.

Ems is the fusion of both eyes granting the user both of the eyes ocular powers hence sasukes ems being a combo of his original ms and itachis as well as madaras ems being a combo of his and izunas

Correct me if I'm wrong... but isn't Madara the only person we've actually seen put in an eye and then proceed to use it?
Madara, Kakashi, Obito, Danzo, Ao, Itachi's Crow, Nagato, and now possibly Boruto.

But of that list, we know that Madara, Kakashi, Obito, and Nagato had literally no recuperation period or difficulty using their new eye immediately.

The very fact that a crow can handle a sharingan at all but Sasuke had to heal for a week after getting EMS is pretty hard evidence to me.

Okay my bad,I wasn't talking about any transplant done during the manga. I was specifically talking about those done on-panel and then just walking it off and or using it. I did forget about Kakashi and Nagato, who are the only 2 on the list besides Madara and Obito to have on-panel (or at least saw them right after). However:

Obito: Hashi's cells

Madara: Hashi's cells

Nagato: Uzumaki lifeforce and stamina

Kakashi: i do have to concede here Kakashi has no break after transplant and he is "normal"

I'd assume that during EMS process, Depending on how it actually works, That one of the set of eyes is destroyed during the process. I don't think it's a simply case of swapping eyes and having your old pair just laying around
This is what I think as well. In fact , I think it is possible that they somehow manage to fuse the two sets of eyes.

How the hell can eyes fuse? What you think they just squish them together and walla new eye? :lmao::lmao::lmao:
Remember that this is fiction. Anything is possible. Somehow they can seal giant beasts of titanic proportions into small human bodies (even the bodies of newborn babies). If that is feasible, then why can't fusing eyes be feasible?

True just look at Kakashi Rin implanted Obito's eyes in him and he used it asap. No recuperation time or anything.

Except the fact that just taking out and replacing eyes don't have a recuperation period. For EMS it does. The manga shows nothing that EMS is a swap of eye's otherwise the ones that tried would have gotten EMS but that wasn't the case even as a Indra transmigrant.
