Sounds like it's not that simple. A way to prevent this to happen: You can only gain EMS if you've exhausted your MS by overusage because otherwise people could just trade their MS to get EMS. Maybe it's this feeling of losing sight in the sharingan that triggers the ability to upgrade your eyes to EMS if you insert new MS?
Sigh.. read the manga pages above, Sasuke said these are your eyes to Itachi. As for why people weren't trading eyes: why would they want to help another person gain power.Okay, for all you guys who can't really think logically: If it wasn't a fusion then why weren't people with MS just trading eyes like candy? It /has/ to be both a transplant+fusion otherwise everyone would attempt to be incredibly OP and there would be no sense of "self sacrifice" or "theft" involved in the manga at all.
The key basics to a story line are development, most of which are done through "reoccurring themes" as seen across the board in all animes. Naruto, the anime, has tons of them; but as described by Madara with his brother it was supposed to be for "self sacrifice" and in sasuke's case "attaining a greater power".
