It's some kind of odd fusion process the manga never bothered to explain. EMS results as the new pair with "Light" if the bond between the two supplying the MSs is strong.

By EMS logic, it shouldn't matter if one or both brothers was blind; if it was a simple transplant, then Madara would have gone blind as soon as he used up Izuna's MS. The EMS is a transformation.
