Sasuke is my favourite, and he's arguably the best written character in the manga alongside characters like Nagato, Itachi, Kakashi etc. But no, he wasn't an underdog at any point. To claim that is just really... fallacious? He was born with the genetics of one of the strongest clan's in the entire Naruto Universe. His brother was a genius and his father was the head of the clan, putting him on a higher pedestal already. After the traumatic events of the massacre, that only amplified how much more powerful he could become because of the inner workings of the Uchiha and how the Sharingan awakens. So from the start he was already gifted with innate ability that his peers other than Naruto didn't possess. VoTE 1 and beyond he either took advantage of people like Orochimaru (CM2, snake powers etc.) or Obito (EMS) or just followed suit the entire time. There was no underdog theme behind him. And nobody thought (reasonably) that Naruto would stomp Sasuke. But the thought holds true that he was weaker than Naruto at the time, since they stalemated when he used the Bijuu's. In their forms without Bijuu intervention, Naruto does indeed win, as the manga clearly portrayed.

For V.O.T.E 1 Sasuke had worse feats and even felt weaker, therefore he was less likely to win. That's why he was the underdog then.

Literally no battle have I seen Sasuke actually internally struggle, though he always had help, either team mate or outside source. He still put up the image of an overall genius of battle.

Sasuke is that person in the moments I mentioned, maybe there's more or less, but those are the examples I saw.
When they were going to fight in part 1, most thought that Naruto was going to win because he was the MC and had better feats, yet Sasuke won.
The same applies to V.O.T.E 2 and when Naruto first got SM. Sasuke was expected to lose, but he didn't.
