In this alternate universe, Sasuke has killed Naruto back at the Valley of the End, awakening the Mangekyou.

The battle plays the same, Itachi is still sick but this time he wants to kill Sasuke, believing he's lost forever.

Scenario 1: Sasuke has Amaterasu and Ribcage Susanoo.

Scenario 2: Sasuke has Amaterasu and V3 Susanoo.

Scenario 3: Sasuke has Amaterasu and Final Susanoo.

Scenario 1-Itachi that weak rib-cage susano would shatter under the constant destruction of the totsuka blade
Scenario 2-Sasuke wins mid difficulty
Scenario 3-Sasuke this scenario was kinder point he would win with ease I think a full susano against a half blind ill sharingan user

Well, what makes you think anything bar V4 Susanoo can block Totsuka? ( which is also a V4 Susanoo weapon ).

Well, after enough hits he could definitely get through, but the thing is, does he have enough stamina at this point to keep it up that long?

Didn't sick Itachi outlast Sasuke at that point anyway?

I feel like stamina is the go-to argument for Sasuke, but he had less endurance back then.

IMO, it can be attributed to his superior chakra-control.

Presumably, when ninja release chakra to mold, it's lost as time goes on.

But since Itachi is so fast/skilled at molding, much less is lost than it is with other ninja.

Control and all that, but Itachi used Amaterasu and Susanoo for a couple minutes and also used Tsukuyomi and a couple katons + one bunshin.

All sasuke did was Oral Rebirth, a couple chidori spears and a few katons.

If Sasuke can't keep Curse Mark and Susano'O in conjunction - that drains his life force and stamina at a significantly faster rate than Itachi's. That restricts Sasuke to either use his Curse Mark, or Mangekyō.

While Itachi has shown to be more victorious than Sasuke with Curse Mark in canon, Mangekyō Sasuke doesn't really stand a chance either if both go toe-to-toe at the start of the match with Susano'O. Itachi has better defense and offense, and after Totsuka pierces, Sasuke is long gone. Itachi wins all scenarios more than not.​

Itachi does have totsuka blade and yata mirror and magatama projectiles to survive an onslaught of kagutsuchi barrages and susanoo sword strikes. While this is happening, he can prepare izanami then gg sasuke. Itachi wins scenario 2 but gets overpowered in scenario 3.
