I think you misunderstood Izanagi.
For the user, Izanagi makes reality of (his death)/(the attack) into a dream.
This happens only when the user is attacked.
Sasuke wasn't hit by an attack from Madara when Madara uses lightning. SO no Izanagi.
When Madara approached Sasuke, he got stabbed before attacking Madara, So no Izanagi.

a perfect example of Izanagi's use was shown by Danzo. And he used whenever he got hurt. I mean the jutsu was active and got utilized when he got hurt.
Also Tobi used Izanagi against Konan when he was hurt. But he used it a little later when he already lost one arm.

If we were talking about teleportation, Izanagi doesn't "teleport" the user, it only destroyes and recreates the instance in another position.

But we have to take care with the definition of the technique. As it is a variation of the Creation of All Things, basically the Izanagi user can do whatever he/she wants. Survive a fatal wound, reappear in other place, and even impale the opponent.

if they were to make a machine that would destroy you and recreate you at a different place with your soul intact.. what would you call this if not teleportation? ^.^
