let me say that if Sasuke with MS fought Itachi with MS. Sasuke would have won. Let me tell you why Sasuke would win with high difficulty. First off Itachi's genjutsu could be easily countered by Sasuke's MS and vice a versa. Heck! Sasuke escaped Itachi's genjutsu without MS so that shouldn't be a problem. Now that pretty most likely rules out all genjutsu. So now it's a full head on battle.
To begin with, Itachi and Sasuke's ameterasu are very different. First, Sasuke's is stronger than Itachi you know it and I know it. Sasuke could change their shape and how they look. Sasuke could easily counter Itachi's ameterasu. Next part off course is Susanoo. Now Sasuke's susanoo and Itachi's Susanoo are different. First of all, Itachi's has the Sword of (i fogot) and the Yatta Mirror. The Sword could seal and the Mirror can block any attacks.
Now i could go either way with this but if Itachi would use Susanoo especially in it's fullest form his energy will be almost depleted. Everyone knows Itachi's chakra is escpecially low so that's why Itachi never use Susanoo as much so if he was to use the Susanoo it will only last only a couple of minutes especially using Susanoo in it's fullest form.
So now the only thing Sasuke has to avoid is the Sword and that should be extemely easy so after Itachi depletes his Chakra Sasuke could easily take Itachi down. I also have to keep in mind that Itachi is smarter than he looks so he most likely try to keep his Susanoo in it's lowest form so he doesn't overuse himself. We should know by now that Sasuke's Susanoo is stronger when you don't regard the Sword and the Yatta Mirror.
Anyway, if Itachi could keep Susanoo in its fullest form for long there is a way to beat the Yatta Mirror. The Mirror only guard the front so all Sasuke has to do is use a weak genjutsu to trick Itachi and attack from behind.
