Hmm possible, but Katsuyu seemed to be confident that she can mend Tsunade's bisection, and Tsunade herself said Katsuyu can do that later but she needs to take care of the other Kages first. Also if she dies wouldn't that make Naruto a little too young to become Hokage.

I remember Oro saying that Tsunade cannot really die in battle and that's one of the reasons he envies her, we'll just have to wait and see.
