Top right panel you see his chest stitched up, Sakura could have easily solo'd honestly. Go into 100 healings and none of those MS ninja tool fodder could've killed her. He probably wanted her to carry out some surgery anyway and let her have a free hit. No other way of explaining how he didn't see her coming. She's slow af compared to Naruto and Sasuke
