So in chapter 638 we clearly saw Sakura stopped using hand seals and she didn't heal anymore

IMO, the position of the hand sign isn't too distinguishable to notice that it changed. It's basically blocked off. I'm just waiting to see what happens.

Also, in chapter 635, she wasn't exactly holding up her hands, healing, either. In a way, like you stated during that moment in chapter 638, she stopped there as well. It's a 50/50 possibility that she either changed hand signs or went back to healing again.

If you're trying to say that she's making the same handseal that Tsunade and Kabuto were making in your illustration, she hasn't changed the handseal, she's went back to making the same sign she was making before.

I doubt it. Even the Slug seemed surprised at the fact that she could use the forehead seal thing, I doubt she trained under them to learn sage mode.
