The lava is formed in the mouth from the chakra network like all other jutsu that is spit from the mouth

Bijuu I would assume need some form of sustenance but they are semi-deities so it could be calorie based or elemental? who knows if it's calorie based then they eat what ever their bodies can ingest... Goku is an ape so he would eat leaves and small mammals... if it's elemental based then Goku eats rocks and stuff
