because his muscles were damaged and then gaara crushed his leg. a better question would be how come gaara turned the rain village ninja into blood rain and only crushed lee's leg you'd think if they were constistent then he'd have taken the leg right off.

To be more blunt gaara crushed it and guy gets muscle strain too.
