Kaguya who is referenced in the Uzumaki Shrine (Three Eyes, The Curtain as her hair, The masks are the souls caught in Mugen Tsukuyomi) had Three Spiral eyes :-

These Spiral Eyes are actually the Rinnesharingan (The base design of the Rinnesharingan is a Spiral). Kaguya was only shown with one (Her Third eye). This Rinnesharingan went from Spiral Zetsu (He himself is a Spiral + Databook hints he is an extension of Shinju/Kaguya just like Black Zetsu [This in turn means you are correct and the plot supports the idea]) -> Gedo Mazou -> Juubi revived in Fourth Shinobi World War -> Shinju -> Madara (His Third Eye) -> Kaguya -> Finally gets sealed away by Chibaku Tensei.

This means the other two Spirals are still out there somewhere.

The theory of mine though based on RT's to solve the mystery of the other two Spirals is that Sasuke has the second Rinnesharingan (His Left eye) and the third Rinnesharingan is sealed in NV's Moon. (This is just a theory for fluff).

No. You are reading too much into this. The eye on her forehead is the reflection of the Shinju's flower's eye. In other words, it represents the chakra she stole. It was a physical mutation she went through after she ate the fruit. A flower is a precursor to fruit. Really, it's pretty simple, straight forward logic.

The Rinne Sharingan is only one eye because it embodies the concept of the third 'inner eye' that allows perception and comprehension beyond the physical and into the metaphysical (or something to that effect; please don't quote me). There's no second eye to go with it, because it's not just an eye. It's the third eye.

Who would Kaguya have take the Rinne Sharingan from, anyway? No one else would have had chakra (or dojutsu, by extension), because she was the sole wielder prior to her sons inheriting it from her. It's more than likely she just developed it once she ate the Shinju.
No it doesn't represent no inner eye.
