To start off, this is more than merely a Genjutsu created in Madara's mind or else he would not need toe Heretical Demon Statue's power to make the stuff in his Genjutsu a reality as stated here. Basically the plan is first to create the Genjutsu, THEN cast it on everyone. That is all Uchiha type powers.
AFTER that they plan to use the sage's Jutsu to breathe life into the dream and turn it into reality. My guess (Using Naruto logic which has worked with things such as how the Rinnegan is obtained) is that once everyone is caught in Tsukiyomi, Madara will be able to attain a state of the entire world as it is in one given instant. Kinda like how it was explained with the Izanami technique where the opponents create a state in space-time...
Once Madara attains a state of the entire world he will be able to use Creation of All Things (A SEPARATE Jutsu ASIDE from Tsukiyomi as clearly stated in this chapter) to rewrite REALITY (Not just the Genjutsu itself since that would all already be within Madara's control because it is in HIS mind). This is why Madara told Obito he'd save the world.
Incidentally, I am starting to believe that the Heretical Demon Statue is acually Hashirama's creation. This is why the Senju cells are so important for control of the statue, and not the Rinnegan (Senju plus Uchiha). The statue seems to be made of wood, which would explain why it can suppress the chakra of the beasts...In other words it was Hashirama's storage system for the tailed beasts when he had them all, before he distributed them across the other villages. The statue allows Madara to create clones of Hashirama because it's Hashirama's chakra to begin with.
