Well seeing how Uchiha and Senju were direct decedents of the Sage, that would but the whole Uchiha clan including Madara, Sasuke, and Itachi decedents, Senju would put the first Hokage, second Hokage and Tsunade, now the Uzumaki clan are decedents of the Senju clan which puts Kushina, Nagato, Naruto and Karin all decedents of the Sage of 6 Paths.

Before the alliances system, the clans were isolated, there was no falling in love with others, there was just war between the clans, so of course they had no choice but the breed one another.

Some clan like the Hyuga remain really closed

Others still perform incestuous but are open

Well by any standard, marrying beyond ur third degree cousins is not incestuous. Also as after marriage the surname changes (In fudal japanese system if a man from lower strata marries someone in higher strata his family name will be changed instead of his wife's), so u cant really tell if one from another. (Except for Hyuga)

Well its incest only when there is a sexual relationship between VERY close members of family, such as *I wont elaborate as there are minors here*. Check the dictionary it says so. Similarly we all humans have same but lil varied 21 pairs of chromosomes and our origins are same, that wld make us all incestous. If it is 'legally' not incest than it can not be 'factually' incest, as laws arise from human morality and social considerations.
