Since they were all former members of the gotei 13 does that mean Yamamoto was the captain commander then or was someone else?

And is the reason why captain Yamamoto wasn't promoted to the royal guard because his power was better suited to proect the gotei 13 or because he didn't do nothing that important?

We yet have to see did all of them served under Yamamoto. For now we only know Kirio served, it's possible that Senjumaru also, considering that she know Mayuri and maybe Tenjiro. Oetsu is at least around 2000 years old and Ichibei is likely first member of RG.
