Why do you think Doctors like to get an arm even if it artificial as fast as they can? A limb stub that is no longer excessed shrinks and no longer can be used. Look it up in the medical books or go see vets or other people who have amputations but never had or wanted to exercise a lim stub. The old saying uses it or lose it. Sasuke has ben without and Arm from chapter 700 were He was 18 years old sines He is a few months older than Naruto. and in Boruto the Movie Sasuke is in His 30. Thats a lot of years without an arm.

Because he didn't want it back.

That missing arm is like a memento of what he has been through (maybe on some level he still hasn't forgiven himself so the missing arm is still punishment in a way), what Naruto did for him, what Itachi sacrificed himself for, and the future he's sacrificing being a halfway decent father to protect.
