Teuchi's hair is allway's concealsed under that hat, So noone knows that it is really spiky, with two "Horns".
He never opens his eyes, so noone knows they are rinnegan.
From the only picture of the sages face we have ever seen(Not including fan art), we can see that it is similar to the ramen guy.

Therefore, The sage used a justu to make himself immortal, and make himself younger. That's right, The ramen guy is the Sage of the Six paths.
