I think Itachi would have remained calm, or lay down and succumb to the disease (remember, he was keeping himself alive with pills and willpower).

However, the only way for Sasuke to die from that would have been from his own plot shield collapsing upon itself due to its enormous mass. Then NV would have imploded.

Really that's like asking what would have happened if Tobi died from Torune's bugs.

Edit: I mean it from the point of writing, Kishi would have never let anything like this happen. I'm not hating/bashing.

Itachi would become the ten tails and solo rampage. Then he would solo all the akatsuki members then solo tobi. You know what, I have yet to understand why this never happened: obito got kabuto/orochimaru to resurrect rin, then fooled naguto or some other person with renningan and give there life up. Or even with Madara. :/ rather start a war
