Question about Izuna Uchiha?

How did Izuna actually die?

Was it Madara removing his eyes or the injury from Tobirama?

Both.

He was dying from Tobirama's fatal wound, and Madara was going blind, so Izuna was like "f*ck it" and gave Madara his eyes, since he was going to die anyway.

He died 'cause Tobirama owned him .

I think it was Tobirama damaging him, and he had complications and one day Izuna realized that he was of no use and decided to give his eyes to his brother in order to fulfill his role as leader of Uchiha, Madara was reluctant at first but then gave away at the idea of getting his eyes. That is my opinion.

Tobirama's attack proved fatal apparently. but it happened off panel. Maybe Madara set it up...

If Tobirama killed Izuna in that fight, then Madara had already taken Izuna's eyes and Izuna was fighting blind.
