when sasuke and madara transplanted their brothers eyes they had to take out their own and replace them right? so could izuna and madara or sasuke and itachi just trade eyes and get ems? how do they do the surgery exactly?

It's my understanding that obtaining EMS requires sacrifice. the brother has to sacrifice their eyes, not just switch them. I'm not sure how that works but that's the rule.

i think they take the chakra im the donor eyes n then place it inside the eyes of the other brother or there's a medical ninjitsu technique for the procedure and i bet its on the uchiha tablet n can b read with a mangekyu sharingan, that or its a specific part ot the eye that they splice into the fortune brother's eyes.

You sound so sure of yourself. but this obviously isn't the case. I'm to lazy to look up scans, but a very common word you see being used between Sasuke, and Obito when talking about Itchi's eyes is "transplant", meaning Obito would replace Sasuke's eye's with Itachi's. If Obito where going to combine the two, I think he'd use a term like splice, or something to that nature. Also when Sasuke fought Itachi, Itachi said Sasuke would be his "spare" eye's, meaning back up, as in he'd still have his. This is just my interpretation of everything.

I think that an eye swap of mangekyou sharingans would work as the sharingan and mangekyou sharingan are the result of special chakra in the brain altering the optic nerve creating the sharingan and mangekyou sharingan.

Kakashi has proven that a transplanted sharingan retains the changes to the optic nerve after transplant so when Sasuke and Madara got Itachi and Izuna eyes they got their bothers mangekyou sharingan.

So I believe that since Sasuke and Madara have the same special chakra in their brain that gave them their own mangekyou sharingan that special chakra would change the optic nerves of their brothers eyes giving the eyes a second mangekyou sharingan creating the eternal mangekyou sharingan.

If my theory about how the eternal mangekyou sharingan is created is right then a eye swap between two compatible Uchiha with mangekyou sharingans would give both uchiha the eternal mangekyou sharingan.

An image of a demon with 4 eyes is shown in chapter 486, where we are told about the secrets of EMS. EMS madara is compared to a demon with 4 eyes, as Itachi is shown with a demon with 2 eyes, having an empty pair of eye sockets requiring Sasuke's to become complete.

Therefore, the manga directly implies that 4 eyes are needed to complete 1 pair of EMS. The wording about 'transplant' implies different, but Kishi seems to have tried to bypass that possibility by making 4 eyes a requirement.
