Orochimaru wins Low-Mid Dif. Genjutsu is no problem, Orochimaru almost broke out of Itachi's Tsukyomi. I don't have the scan of him trying to make hand seals when he was in Tsukyomi though :I

It wasn't a "Sneak attack", And Oru clearly just wanted Itachi's body it wasn't even a fight. Oru literally did not do anything and Itachi the same. It was a small skirmish and nothing more. The Genjutsu could of simply been oral rebirth no diff if he was serious.
