Pinhead is your average sociopath, socially inept, serial killer. Black leather trench coat, extremely pale skin (due to being stuck indoors for long periods of time dismembering innocent children), and a large quantity of pins stuck in his face in a grid pattern. Quite ordinary, you see these guys every day.
So you think you can understand me?
What more could you ask for? The guy is crazy, and longs to remove the flesh of the innocent while they're alive. He lusts for blood, he lives for death. His mind has one focus, and that's his next victim. Of course, under all that murderous insanity, there is a warm glow. A special somebody who stirs his loins. That person is Urumi Sanza.

Parents tell my story to scare their children.
Not many know Pinhead's true story. Those who do, know to fear it. There are many perverse stories told, some that glorify and others that demonize Pinhead. The truth is, he was born into a dark and gloomy world. His family was killed by ninja in a great battle. He was left orphaned and destitute. (Having a dead family is a requirement for a bio, right?)
As a child on the streets, life was amazing. He had nobody to tell him what to do, and so he did as he pleased. He started killing small animals to feed himself. Soon he killed a dog. The canines' constant whimper encouraged him. He loved the sound it made as it begged for it's survival.
Soon he was killing people's pets, leaving their organs strewn about their yards. It was only a matter of time until he turned on a human boy. The villagers looked for him for days. He was never found, not even his bones.
Before long, children like Pinhead who had no family were forced to choose professions, most going into the ninja academy. The ninja academy was the choice for Pinhead too. Not so he could become an amazing ninja, but so it would be easier to murder hapless people.
As he grew, and became a ninja, so did his murdering prowess. He found it easy to capture victims, and mutilate them in secrecy. It wasn't long before he left his village's borders and wandered aimlessly.
One night, became the most amazing night for him. It was the night a fire was lit in his heart. He was sitting by a camp fire, eating the flesh of a young mother who had recently conceived a child (which he had already ate). Yet, as he bit into the raw flesh of her thigh, a young woman stood completely silent and mesmerized by the site with a decadent look of cannibalistic hunger in her eye.
The hungry girl was attracted to the flesh, and disregarded him completely as she grabbed a bit of her own. It was love at first sight. He didn't address her immediately, there was plenty of human flesh to go around.
Though afterwards he spoke in the most haunting of tones. "I wasn't expecting dinner guests. To whom do I owe this splendor, this graceful woman who dines before me?"
As she finished eating herself, she grunted, and gave a simple one word response "Urumi."
From then on, they would be companions.
He soon found his way to Mount Myobokuzan, where there were toads everywhere. His first thought was to kill and dissect one, yet something agitated him. He knew he wouldn't be able to defeat them. Instead, he greeted them and swore his allegiance to them.
As life went on, so did his ventures. He delved deeper into the darkness, subjecting himself to madness and began permeating with masochism. His body was mutilated as he forcibly drove pins into his skull. He explored different lifestyles but nevertheless returned to the dark, subjugating himself to the shadows.
