Kabuto just needs a small sample of DNA then the Edo Tensei takes that form with the help of a human sacrifice.

Ithachi had his eyes when he died. They take the same form from when that person died.

no, not there death but there prime, madara was prob old when he died but came back in his mid 30s like around when he fought young oonoki, also the 1s and 2nd hokage were younger then hwen they died

If zetsu was around watching the battle he is the battle he could have taken enough DNA when taking back rings and making sure bodies were not found. . Also kabuto was a spy in akauski
. He could've taken when he needed beforehand.

How do you know the ages of the first and second hokage died? And that when they were brought back they were younger?

It is generated from DNA so I guess it retains the body memory from when the DNA sample was obtained
