So how did she actually die? I thought itachi would kill her and put her in a genjutsu showing her whole life in 1 second.
According to the Novel, Itachi met up with her, and casted a genjutsu on her, in this genjutsu, Itachi and Izumi got married and had kids, they eventually grew old together and died of old age, after the genjutsu, Izumi says "thank you" with tears streaming down from her eyes and dies in Itachi's arms.

Believing that ending her life first would quell his inner doubts, he proceeded to kill her and her mother first. You sure? I mean so were the shinobi Itachi killed but either way Danzo would have been trouble and Sasuke would have been even more pissed if Danzo actually took his father's Mangekyou.
Hope they show it as a flashback before the Sasuke vs itachi fight when he is about to die. Would suck to miss this opportunity but from what I've seen so far they aren't really bf and gf
remember when his dad was telling him about the mangekyo sharingan during their talk that hinted it right there and i doubt itachi could defeat hus dad at the time
