We do not know when naruto became hokage. We only know he is hokage during the meetings and the manga does not say when the meeting in the flashback occurred. People are using kakashi's words in 700 to form a timeline. I do not think sakura should be pregnant at the time of the meeting. But I do have to admit she looks worn down in that picture and her belly kind of looks bigger. We will have to wait and find out.
