We know Izanami caused loop.Is that all happen in Kabuto's mind or the loop actually happens in real world?

We saw in the end Itachi holding onto Kabuto's head, probably to show us that he's using Izanami through Kabuto's mind. Is that also mean that Naruto & bee are not in 'paralyzed' situation (like what people have complained)?
