If he teleports a part of his head into the Kamui dimension, he's effectively cut off part of his brain and is now brain dead.

It's not really that simple. I believe it's called Spatial Manipulation, he basically bends the boundary between Kamui world and real world around himself. It's more complicated than Kakashi explained.

but he didn't =/ I just find it weird how he "teleports" to another dimension yet his whole body is still visible. Naruto experienced phasing through him, but never noticed any part of his body removed from vision.

yeah the weirdest part is that he is still visible even after his head goes in kamui land but manga says he teleports exactly when opponent strikes so although it seems impossible but may be he times it perfectly...
OT: only explanation could be that he keeps the portal open, which was already given by someone above me...

his body is sort of "always" in the dimension. all he has to do is solidify an arm or leg to have a physical presence.
i assume that when he phases its all still linked biologically
