Obito took either Old Madara's eye or some Uchiha, then after using Izanagi, he took Madara's Rinnegan and then got back his original eyes.
That's three eye changes.
Sasuke only took Itachi's.
And Madara only took Izuna's at the behest of his brother to protect the Clan and later His Right Rinnegan from white Zetsu, then Obito's left eye to use kamui and get his left Rinnegan.
Itachi changed no eyes, though.
