Look closely at the last page of 601. Madara looks pretty much like Nagato after using Rinne Tensei. And also he's telling that Obito's between This and Another world. Perhaps Madara'd used Rinne Tensei on Obito?

#1 Madara looks like that because he is OLD.
#2 If Obito was Rinne'd then shouldn't he have been completely revived? As in, no need for surgery. Example: Pain resurrected the Asura Path when he was utterly destroyed. Asura came out completely revived even having his robe returned.
