The last time we saw Tsunade she was cut in half and said that she could save the 4 other unconscious kages. Many including myself believe she will die after saving them but some think because of what the slug Katsuya said she could still survive. Still even if she somehow does save herself I do not believe she has long to live. When Tsunade first used her forehead seal that heals her damaged cells Shizune commented how it shortens her lifespan because her cells can only divide so many times so she she only use it if absolutely necessary. In the Pain arc she used it to replenish her chakra and when it ran out she went unconscious and her body got really wrinkled and looked old. I believe her body looked so old because of all the times she used her regeneration jutsu. So really Tsunade normally uses a jutsu that makes her look like she is 30 when she is 55 but her underlying cells really are the age of someone who is 80. During the war she first used the forehead seal to repair her cells after she teleported to the Madara battle. Then she used a repair jutsu that kept repairing cells after she got stabbed and burned against Madara many times. Now she is sliced in half and in order to survive will have to repair her cells even more. I know this isn't real life and Kishi doesn't have to even follow his own logic but if he does I believe Tsunade will die soon if not from her injuries then simply to equivalent of old age.
