I dont believe that would br the case, before people had the luxury of cars and travel they mated with their clan and I dint think they were mentally challenged

I still hope naruto gives up on sakuras stupid ass and gets with hinata. shes strong, loves him completely, hotter than sakura, and submissive (best quality, she would let him do ANYTHING to her) compared to sakura whos over aggressive and dominating would probably try to use strap ons and such. and shes abusive as all hell.
