How did it make sense that naruto's shoulder didnt heal itself. His cloak in one tails healed almost instantly after sasuke stuck his arm through his heart with his chidori. and in with the four tails cloak he resisted being pierced by the kusanagi blade, how does it make sense that now when karuma wants to protect him, his shoulder gets dislocated... (I know its after the fact, but i never understood it.)

It was a plot device back then.
A plot device that quite frankly Naruto doesn't need anymore.

I think he can only heal his wounds. Not fix broken bones and stuff.

the bone wasnt damaged, it was dislocked, there is nothing to heal there.

Well those you mentioned where physical injuries that required tissue re-composition to heal and I assume that Kuramas chakra accelerated the process.A dislocated shoulder need pressure to a particular spot to be relocated.Naruto could have used a chakra arm to do that by hilself it just happened that Hinata saw that and fix it.

His shoulder was dislocated, not torn/bruised/ ect. It needed to be put back into place manually in order to heal. If you dislocate your shoulder and don't place it back into the socket, it won't heal. Same for Naruto, he just heals faster.

Even Superman can't use His full strength if His shoulder gets dislocated. Kurama heals chakra and mussels, a dislocated shoulder like a broken limb must first be set in place to be healed right.

If it doesn't heal dislocations then I wonder how Naruto survived Sasuke's piledriver in part 1.

His bones can't move by itself, he is not kimimaro. It was simply out of place, there was nothing to heal. It was simply a case of putting the bone back into position.
