Intent to kill does make a difference because if you don't have the intention of killing someone, it means you are holding back in regard for their life.
However, in this case, it's wrong to say 'if Naruto had intent to kill' because Naruto gave it his all anyway with the intent of staying alive and defeating Sasuke, because if he continued to hold back, he would have lost.
