There are many characters which ain't perfect. This is in every Manga the same. The same in Naruto, in Dragonball, in One Piece or elsewhere.

Not in the first one. Inorganic Reincarnation would kill an alive Itachi. His Sharingan wouldn't be able to see it coming.

Naruto's Sage Mode on the other hand, can sense living things, so he would be able to dodge it.
