I think Madara had an intentional factor in Rin's death. Also, I think Kakashi accidentally killed Rin because he though she was an enemy (maybe a genjutsu) because killing her intentionally would seem to contradict Kakashi's decision of adopting Obito's quote of those that abandon their comrades are worse than trash (unless, of course, Kakashi later adopted that in reflection and was even talking about himself).

Dude, did you read the recent chapters? Kakashi fired up a Raikiri and Rin came in between the Raikiri so that Kakashi would kill her unintentionally and her death would save Konoha from being attacked from Sanbi. Even though it was not Kakashi's fault, he still blames himself. Obito referring to Kakashi as trash means that the shinobi system had made him trash.
