chap 540 name is "" MAdara's plan"" ..so in page 3 nagato refered tobi as he is madara and itachi says that u r right nagato..so both itachi and nagato convinced that tobi is MAdara...and we know that he is not madara...and question is why kishi made this chap named Madara's plan..he must have do it like Tobi's plan ... now this is just mistake of kishi when he thought of chapter name or just he also dont know who is tobi??

The 6th coffin wasnt revealed to be the real Madara for a good 15+ chapters after 540. Kishi named it "Madara's plan" to further push us to believe Tobi was Madara, for the shock value to come.
