I know many people are speculating about "Madara couldn't give Nagato his eyes, it's a plot hole due to the timeline, FFFUUUU..." etc.
But why do they (you?) think that "gave" nessesarily means "transplanted"? Let's see what we know:
1) Transplanted eyes (sharingan, rinnegan) are not capable of being turned off. The're constantly active.
2) We've seen Nagato with his eyes not-awakened during his childhood. We've seen him awakening them in manga.
3) How do you see transplanting fully grown-up man's eyes into the little child's skull? The're not matching the socket's size :)
4) And here we can see that Madara and Nagato look pretty familiar:

.

So, why saying that it's a direct change, implanting one's eyes to another man? Why can't he be his son, for example? :)
Hashirama's wife was Uzumaki. Sure we don't have any info about Madara's private life, but why shouldn't allmighty Head of the Uchiha have his one lineage? :)

Madara could have removed a sealing Jutsu place on the Uzumaki at birth to prevent any Uzumaki from ever developing a Rinnegan. The war that destroyed the Uzumaki village could have ben to prevent any future Uzumaki from unsealing the Rinnegan. It's a possibility to consider.

When we see old Madara, he has just the 3 tomoe sharingan... who says that his sharingan in that picture weren't a spare pair he got from a dead Uchiha which he transplanted into himself after giving Nagato the Rinnengan?

madara focused all his chakra in awakening sharingun, mangekyou, EMS and rinnegun, so he couldnot focus on developing his lower parts... hence he couldnot have had a child. a possible theory... LOL

Since you obviousely didn't read my topic-starting post, I'll repeat it here:

I hope you're kidding. I was about to awaken my MS but if you are right I'll quit. **** world domination, I just wanna ****.
On-Topic:It's possible for Nagato to be Madara's son, I mean, all he had to do is to put Nagato's mother under genjutsu for 5 minutes. Then bang-bang.

See: all people of male gender here please stop awakening your MS

(And soon the nine tails will be ours , ladies!)

plus I dont think Madara is Nagatos father and that is how he inherited the eyes.

Wow. Eyes never grow they stay the same size from birth to death. i doubt the sockets get bigger. so adult eyes into child sockets is not an argument or point.

No, u r wrong, sir :) As a bachelor in medicine, I can assure you that from the babyhood to adult eye's diameter changes from about 17mm to 25mm, and the crystalline lens continues to grow up throughout your whole life. The same goes for sockets :) And the pupil of the eye begins to decrease in size from about 20 years.

A babies eye grows by 28% til it becomes an adult eye. The majority of the growth occurring at a very young age. Nagatos eye sockets would likely be able to just about a fit an adults eyeball (and for all we know, he has abnormally large eye sockets.)
I'd be surprised if eye socket sizes were on Kishi's mind...
Those two points could explain your 3rd point.
2nd point, While i might be forgetful, we've seen Nagato without the Rinnengan when hes younger, but we've never seen them deactivated have we?
1st point, same as before, I can't remember Nagato deactivating his Rinnengan, so if theres a picture that shows otherwise, let me know.

We can't say what Kishi's thinking about, but I doubt he would've done such a mistakes. Don't you think he'd creating the manga solo, without anyone consulting and assistance? Lol.
About his childhood...
During the Second Shinobi World War, Nagato lived with his parents in a village somewhere near Amegakure until one day when two Konoha shinobi broke into their home, in search of food and believing it was abandoned. Nagato's parents, intent to protect their child for who they believed to be killers, died attacking the Konoha ninja who mistook them for enemy ninja. In his grief, remembering his parents' death since as the first time he felt immense pain, Nagato awakened his Rinnegan for the first time and killed the assailants.
It's the manga. And when he was a child, his eyes were always hidden by the hair. But it's a clear fact: he's -awakened- his eyes when he was child, they weren't activated from his birth.

I will admit now that I've read the manga chapters, he does kill them, seemingly with his Rinnengan. However, for all we know, Madara had done something to him before hand, which would have them activate like the sharingan does, under great danger or stress.
It will likely be explained.
I'm sure I could give a half decent argument for Madara doing the transplant, as it could be possible, with med nin techniques and one of the most gifted ninja ever. Also, Madara might of just made been in the background, pulling the strongs to make Nagato unlock it.
But from Tobi and Madara, its clear that they very likely were a catalyst in nagato gaining the rinnengan

Point #3: It's just a manga, stuff like that isnt important. Itachi implanted Shisui's eye into a crow, that wouldnt fit in real life either, right?

That's very plausible, Madara + Uzumaki wife = Sage of Six Paths, so Rinnegan...
That's why Tobi called Nagato the 3rd Sage Of The Sixth Paths, maybe because his DNA is from uchiha and from uzumaki...

Maybe Nagato is a direct descendant of Madara + Uzumaki which would then make sense as to why he said he gave Nagato the rinnegan.

A sharingan evolves in a rinnegan, right? then I'm really wondering how Nagato activated his Rinnegan without ever having had a sharingan?
It must have been implanted eye's with a trigger (like stress, anger, despair) those eye's had the Rinnegan unlocked before or else it would be impossible for Nagato to unlock it bcs he had no sharingan.

quick thought :D
