Now, this may be flawed, & I know that, so please, shush.
First lets start with some factors

Basic Hollow Ichigo
Now onto my theory.
Tensa Zangetsu is Ichigo's original Shinigami power, before the Hollow fused with them, Tensa Zangetsu, before the Hollow came was Ichigo's base power, makign it's Bankai the simple Shikai, leaving an unknown true bankai, that true bankai takes the form of Ichigo's Valsto Lorde form, but not the same sword, it instead used Mugetsu, However, when Ichigo's power fused with the Hollow (in Urahara's training area), it replaced Tensa Zangetsu's bankai (Mugetsu), & made it an unstable attack after its reiatsu was scrambled after the Hollow entered, making Tensa Zangetsu the bankai & making a Shikai out of the hollow blades that he had on his hands during the flashback.
Mugetsu wasn't rendered completely unstable, which is why he was able to use it, but Zangetsu knew that if he used it, it would cause him to lose his powers, so Zangetsu (now fused with the hollow) , had to forge a completely new attack, the "Getsuga Tenshou", not as strong as Ishinn's because of the half Quincy inside which now weakened Ichigo's power. Then Juhabach and his entry comes into play, while Ichigo is fusing his power into the hollow during Urahara's training, Juhabach's spirit found his way inside Ichigo VIA his broken soul chain, How his soul came about, no Idea. Because Juhabach is stronger than Hollow Ichigo, he subdued him & his shikai abilities & then absorbed them, but through the Tensa Zangetsu, (Now the Bankai), he would still be able to show himself & a false version of his shikai, and a real bankai, which explains why he was able to fight Ichigo during his training with the Vizards. Fast forward to the Arrancar saga, we see Ichigo using his mask, the mask is the result of his bankai fused with the Hollow, which is why Ichigo went Vasto Lorde against Ulquirorra, Ichigo was killed which ment for that time being, his Shinigami powers were Nullified & allowed his hollow to assume the Bankai & go full hollow form, Much stronger than his full hollow form he took againts the Vizards because his Shinigami powers were still active to keep the hollow in check, as Ichigo was unconscious, not dead. After his full hollow form ran out of energy against Ulquiorra, all the hollow could do was revive him with high speed regeneration, allowing his Shinigami powers to activate again, & because he was in Vasto Lorde form when his power ran out, he was given a new mask.
Skip to Ichigo's dangai training, we see the hollow in Vasto Lorde & Tensa Zangetsu, not because he is in Bankai, but because Juhabach's power wasn't able to take as much control, leaving them time to train, why Tensa Zangetsu told him otherwise, is likely because he didn't want him to know that he had Quincy powers. Tensa Zangetsu & the Vasto Lorde fused, they were able to supply Ichigo with enough power to learn & use Mugetsu, (The original Bankai), just once, they lied & told him that he would lose his powers so that he would never look forward to fighting again, for his protection, & so that he would be careful with the attack, in reality, his powers just needed to recharge.
Fast forward to the Fullbring arc, Kugo teaches him Fullbring, Ichigo unknowingly uses his Hollow powers & Shinigami powers, which is why he could use Getsuga Tenshou, & why he could go into Fullbring form, he had used up the energy that was recharging, but not enough to make it go dry, when Kugo stole Ichigo's power, he only took the power that was recharging, not the whole package, as explained by Rukia, & teaching Ichigo Fullbring gave him another row of power allowing him access to 4 powers rather than 3, which mean that when Ichigo's power was restored by the Reiatsu of the blue sword, it gave him a new Shikai & Bankai, it spread the Reiatsu evenly among the 4 powers, his 2 new sword forms being supported by the Hollow & Shinigami fuse, his Black Coat thing being the Result of Quincy powers, & his neck brace & gloves as a result of his Fullbring armor, he no longer can use his mask or Mugetsu, because the natural selection of his powers discarded them both, although he still has the potential & ability to relearn them both, because both sources are still there, & that also means that Ichigo's hollow could have a new form, the sign being that has swords have changed, but I'll save that for another theory....
