Now that we have more insight on what the Otsutsuki Clan represents. I now have a pretty comfortable theory on their goals, intent AND ultimately Kawaki.
So far we know the following about the Otsutsuki Clan:
1) There are 3 new Otsutsuki.
2) Kaguya was a rogue Otsutsuki
3) Momoshiki and Kinshiki are under their clans orders to find worlds to harvest.
4) They believe in passing their strength onto the next. I.E converting each other to fruit or even passing their chakra to their children. Something Kaguya selfishly decided not to do.
With the 4th reason alone it kind of validates that there are Otsutsuki that will be stronger than Kaguya. But lets continue to my theories
Little is known about Kawaki but here are things that stand out about him:
1) He has curse mark similar to Boruto
2) He vaguely hints at multiple dimensions(Which are already confirmed)
3) He wants to defeat Boruto
With this information and based on this manga scan:
We can conclude that Kawaki potentially defeated an Otsutsuki similar to Boruto which granted him a curse mark as well. With this information in mind here are a couple theories I have about Kawaki
It seems that the Otsutuki are a quite interesting group but ultimately I believe their clan ways will be their undoing. The clan's culture is self destructive which was showcased with Chakra obsessions and the willingness to harvest one another. Should be interesting to see how it all plays out.
I know some may dislike the idea of how the series is no longer simply about Ninjas. I'm actually glad the series is trying to emphasize every aspect of the series. Jutsu, Chakra, Dojutsu was never explained especially when you consider that chakra comes in variety, potency and levels. We finally get to see the origins of chakra and its impact on a world unknowingly using it.

1st Theory: Boruto is cursed to become an Otsutsuki
After Momoshiki's defeat, Boruto was given an incredible power that he himself doesn't understand as well as a special seal in the palm of his hand. I would like to emphasize that Boruto gained this by defeating Momoshiki
To me this would indicate that Momoshiki or Otsutsuki have some form of respect within their clan that once they're defeated they would rather transfer chakra than see it destroyed.
But more importantly, I believe this seal is what converts people into Otsutsuki. I say this because Momoshiki stated:

Main Theory: Kawaki is from another dimension and wants Boruto's power for Revenge on the Otsutsuki
This may come off as far-fetched but it sort of makes sense if the Otsutsuki clan goes around from dimension to dimension harvesting worlds. What I possibly believe is:
Kawaki defeated an Otsutuki in his world but then lost his world due to an even stronger Otsutuki(Leader Maybe?) To avenge his world or prepare when the next Otsutsuki leader comes he requires Boruto's curse to be capable of fighting the next threat. I guess you could say its a Sasuke avenger approach where its do whatever it takes to get revenge no matter how drastic the actions are. Rather than work with Boruto, Kawaki would rather do it on his own. This has been done more already when you consider Kaguya used Zetsu army AND harvested chakra to prepare to fight other Otsutsuki Clan members.
Also lets not forget Sasuke fought Naruto at VOTE 2 for the same exact purpose. Shoulder everything on his own.

Naturally, if there are 3 and the 3rd has remained hidden its quite possible that the 3rd was defeated by Kawaki at some point. It could explain how Kawaki got his curse mark but I don't believe all 3 would enter earth.
Possibly one of the side effects to gaining the curse mark is an obsession for chakra. For all we know the Otsutsuki threat could be over by the time the Kawaki and Boruto fought. Now its simply a battle to be the strongest.
