So I've been wondering, is he a transgender or a girl who was sexually abused in her youth and because of that she started living as a man out of fear of having to experience anything similar again?

Lol it's not just that I think of it, it's preety much a fact, it's just a first for me to see them bring up at a manga the sexuality of someone and in such a confusing way so it triggered my interest.
