It was implied it doesn't need to be stated. If something is noticeably loud or irregular Tsunade notices it, she has done this analysis to no one else. Erratic doesn't = loud.. no clue where got that from. You can only tell if the heart beat is erratic if you can hear it. You have no evidence to state that she can't hear a normal heartbeat.

Ok then, what is a regular heartbeat? And what is a erratic heartbeat? Tell me, since you obviously know that the only way she can hear a heartbeat is if its erratic, you should be able to tell me that specific patterns she can hear. And what the regular heartbeat is that she can't hear.
