My Opinion + Argument:​

Sasuke takes this. Once Susano'O comes up, neither has anything that can reach the other. Sasuke has the upperhand here due to his superior stamina, add the fact that any Amaterasu used increases his chances of forming Kirin.

Genjutsu is useless as well, Sasuke breaks any of Itachi's Genjutsu including Tsukiyomi. As it was stated that he broke it with his own Sharingan. The "Itachi was going easy on him" argument falls flat here as well, Itachi had to make it believable or Sasuke would know something was up, and since then, Sasuke's own Genjutsu prowress has increased along with his Sharingan advancing a level. The same goes for the Curse Mark argument as he never actually used the curse mark, it only showed up in the Genjutsu and was never shown in real time, hence, it was only part of the Genjutsu.

Itachi wins.

-Already outlasted Hebi Sasuke during their fight, so if it comes down to attrition, as Itachi has no illness, Sasuke will run out of chakra first and will be forced to put down Susanoo. Then Itachi one shots him.

-His Susanoo is a stage above Sasuke's at this point. Sasuke only has Complete while Itachi has his Armored Complete Susanoo.

-Kirin is a once in a battle type jutsu so once its used it isn't happening again, and it isn't going to get past an Armored Susanoo when it failed to obliterate Itachi due to the protection of his Skeletal Susanoo.

Could go either way, but I am leaning towards Sasuke high difficulty.

-Itachi whether being healthy or sick is a pointless condition. We don't know how long he can activate his Susanoo, and we don't know how long he can last. It would be a mere speculation and pointless. Therefore, we can only rely on sick Itachi's feats.

-MS Sasuke's final form of Susanoo wields an Enton. So in fact, he can extend his Enton. Furthermore, he can armour his Susanoo with Enton armour which increases it's defence.

This fight would only come down to Susanoo. People will say Itachi's superior due to its baseless hype. Totsuka cannot penetrate through Sasuke's shielded Susanoo, and Sasuke cannot penetrate through Itachi's shield. So the fight will come down to who gets outlasted. As I said, we can only rely on sick Itachi's feats, therefore, Sasuke will eventually outlast granting him victory.

Itachi wins high difficulty.

You guys are wrong when you say "it all comes down to susano'o". Itachi is not the type of fighter that spams MS with every attack he uses, that is Sasuke. I agree that genjutsu will not work here. But Itachi has more than just his MS power.

Itachi can use Kotoamatsukami, and trick Sasuke in to suicide. Itachi can use Izanami, and make Sasuke accept the truth of Itachi's own sacrifice was for the greater good (this is the Sasuke that fought Danzo, the one going on a konoha blood thirst).

As stated before, Itachi and Hebi Sasuke's stamina both fell at the same time. Kirin did Sasuke in stamina wise, but it's not like MS techs wouldn't either. Remember, Sasuke spams MS, Itachi does not. Itachi has the best jutsu analysis out of any character, he made Nagato seem easy. The only thing giving Itachi trouble here is Sasuke's magnificent control of Amaterasu. Even so, Itachi will know how to break it.
