MS/EMS question

Ok so i've recently starting watching naruto and I've been wondering about MS/EMS.
My question is: How does EMS happen by just trading eyes?
To be more specific, What I mean is, how is it possible to have a new light by just simply trading eyes or obtaining new eyes?
This is more of a sasuke/itachi question.
In the past kakashi asked itachi about his eyesight and all MS users eyesight worsens after a while, even sasuke himself started getting really bad eye sight. So this is where my question comes in. When itachi has bad eyesight or at least some kind of damage to them and he dies by sasukes hand, how is it possible that that bad eyesight isnt passed on to sasuke because he does have itachi's eyes after they were implanted into him. How do they become eternal MS?
I find it kinda weird that just changing eyes would make it eternal and it kinda makes no sense unless i'm missing something.
If anyone could explain then i'd appreciate it.

that's the thing about the sharingan... its activated and enhanced through the pain of the user... you obtain MS by killing a best friend... you get EMS through pain of losing a brother (also the fact that you get MS+MS)... thats why the uchiha history is tainted with blood and all that jazz...
